NR670.042 (2)(b), Wisconsin Administrative Code, that Badger Disposal of WI., Inc. has applied to the Wisconsin Department of Natural Resources for a Class 2 modification to its Hazardous Waste Storage License. This modification requests Department approval for lab pack operations in an existing building recently acquired and renovated, approval of changes to the shipment screening procedures described in the facility waste analysis plan and approval of changes to secondary containment methods for containers. 13 - Notice of Complete Application for Proposed Riprap JNA Staffing, Inc,1317 West Lincoln Ave, Milwaukee, WI 53215 and Blackhawk Development, LLC have applied to the Department of Natural Resources to install riprap on the southern bank of Burnham Canal (Menomonee River). The proposed project is the stabilization of 11th Street parcel by regarding and shaping the bank and then placing riprap The project is located in the Northwest ¼ of Section 32, Township 7 North, Range 22 East, City of Milwaukee, Milwaukee County. The Department has determined that the application is complete and is currently evaluation the applicant's proposal. The Department must consider factual information about the following legal standards in deciding whether to issue, modify, or deny the approval or permit to the applicant: Whether navigation is materially obstructed, including commercial, recreational, active and passive forms of navigation Whether there is detriment to the public interest, including fish and wildlife or their habitat, natural scenic beauty or water quality Whether the flood flow capacity of a stream is materially reduced The Department will follow the steps below to reach its final decision on the application: 1. Review the plans and information provided by the applicant. 2. Review the information from public comments. 3. Review the information presented at a public informational hearing if one is requested. 4. 17 - City Ordinance 116-8, and City Charter 11-24, which states, in part: "The owner, occupant, or person in charge of any real property abutting, or fronting, upon a paved public sidewalk or crosswalk shall remove and clear away or cause to remove and clear away, all snow and ice within 24 hours after the snow and ice has ceased falling." If a person is found to be in violation, they shall be immediately subject to a special charge of $25. In addition, the first violation following the issuing of a sidewalk snow/ice clearing notice will result in a special charge of $45, plus removal costs. The second and each following violation will increase the special charge to $90.

19 - OAK CREEK-FRANKLIN JOINT SCHOOL DISTRICT Board of Education Meeting - Condensed Minutes October 12, 2009 1. Meeting was called to order by President Frank Carini at 7:00 p.m. 2. Pledge of allegiance was recited. 3. Roll call was taken. All members were present. 4. Consent Agenda #8648-1009 Mr. Mason moved and Mrs. Cerniglia seconded to approve Consent Agenda items a, b, c, and d. Motion passed unanimously. a. Approval of meeting minutes for the Board of Education meetings of 9/14/09, 9/21/09, and 10/5/09 b. Approval of agenda for the October 12, 2009, meeting c. Approval of submitted lists of bills and payrolls d. Approval of personnel matters 5. Information Items: a. Recognition of East and West Middle Schools as exemplary middle schools in Wisconsin: The Association of Wisconsin School Administrators recognized East as an Exemplary Middle School for having reading scores in the top 10% of the state last year, and recognized West as an Exemplary Middle School for the significant growth made last year in reading scores. Pete DeRubeis, East Middle School Principal, and Michael Maxson, West Middle School Principal, accepted the recognition from Mr. Carini and acknowledged the efforts of their teaching staff for achieving this recognition. b. Recognition of National School Lunch Week: Superintendent Dr. Sara Larsen read a proclamation from State Superintendent of Schools Tony Evers proclaiming October 12-16 National School Lunch Week. She recognized the efforts of all food service staff for providing quality lunches to district students every day. Ms. Bernadette Rein, Taher Food Service Manager, accepted the recognition and noted that the district is now participating with the Wisconsin Growers to provide fresh produce from area farmers for use in preparing school lunches. c. Presentation on Assistive Technology: Cyndi Kieck, Special Education Coordinator; Sara Schiesser, Occupational Therapist; and Cathy Thiry, Speech/Language Pathologist, presented information to the Board on the efforts of their committee in providing different pieces of equipment to meet various student needs. They demonstrated a range of items available that are distributed to students based on age and need which assist with their communication and literacy. This team is working this year to further develop the implementation of the use and distribution of assistive technology equipment. Funding from the American Recovery and Reinvestment Act (ARRA) is being used for this planning as well as for the purchase of additional necessary equipment. 6. Public Input: The Board received input from a representative from ABM regarding the process and result of the district's solicitation of quotes for cleaning services and the contract to be approved later on in the agenda as a result of this process. Reconvened formal portion of the meeting for Board discussion and/or action. Business Items 7. Old Business - None 8. 26 - STATE OF WISCONSIN DEPARTMENT OF NATURAL RESOURCES PUBLIC NOTICE OF INTENT TO REISSUE A WISCONSIN POLLUTANT DISCHARGE ELIMINATION SYSTEM (WPDES) GENERAL PERMIT Discharges Covered by General Permits: Point source discharges, located within Wisconsin and meeting each of the applicability criteria of a general permit, may be covered under the general permit for authorization to discharge wastewater to surface waters or groundwater of the state. Each proposed general permit contains requirements designed to regulate pollutants commonly found in these types of operations. The proposed general permits have requirements for discharges that assure compliance with surface water and groundwater standards. When a proposed discharge contains pollutants that are not typical for the general permit category and the pollutants could exceed a water quality standard, the Department may issue a site specific WPDES permit that includes any additional limitations needed to regulate the discharge. Brief description of the general permit proposed to be reissued: Facilities Discharging Domestic Wastewater to Groundwater via a Subsurface Soil Absorption System (WI-0062901-2). The proposed permit is applicable to new or upgraded private onsite wastewater treatment systems (POWTS) if the design capacity of the system is defined in accordance s. NR 200.03(3)(d), NR 200.03(4) and NR 200.03(5), Wis. Adm. Code, to be greater than 12,000 gallons per day, or serving more than the equivalent of 85 residential bedrooms. This general permit will be issued to POWTS owners that meet the applicability criteria in the permit and are approved by the Department of Commerce under COMM 83, Wis. Adm. Code. A publicly owned treatment works (POTW) that discharges to groundwater via a subsurface soil absorption system may also be covered by this permit. The permit applies to treatment systems that are designed and operated to remove nitrogen to the extent technically and economically feasible. The proposed general permit would not normally be issued for any system located in a "source water protection area" for a public water system well, but the Department may, on a case-by-case basis, waive this requirement. A "source water protection area" is an area, within which certain facilities or activities could potentially affect the water quality in the public water supply well. The proposed permit would require each system to be operated and maintained in accordance with a management plan approved by the Department. Summary of the General Permit Program: Section 283.35, Wisconsin Statutes authorizes the Department of Natural Resources to issue general discharge permits for categories of point source dischargers. It is more efficient for the Department to cover multiple facilities under a general permit rather than issuing individual permits for each facility when no special circumstances warrant site specific permit requirements or limitations. The general permit program is intended to minimize paperwork for the permittee as well as the Department. However, if the Department determines it is necessary or appropriate to withhold or withdraw a facility from coverage by a general permit, it may do so for the following reasons: 1) A facility may be removed from general permit coverage and issued an individual permit at the written request of the discharger. 2) A facility may be removed from general permit coverage and be required to apply for and obtain an individual permit. Any person can submit a written request that the Department take such action. An individual permit may be required if: a) the point source is a significant contributor of pollution; b) the point source is not in compliance with the terms and conditions of the general permit; c) a change occurs in the availability of demonstrated technology or practices for the control or abatement of pollutants from the point source; d) different effluent limitations or standards are promulgated for a point source or class of point sources covered by the general permit; or e) a water quality management plan is approved and it contains requirements applicable to a facility covered by the permit. Intent to Issue General Permit: The Department has tentatively decided that the general WPDES permit described above should be reissued. D. 3120-01-30/60 WIS 67 Pavement Maintenance Project Limits: WIS 59 - US 18 Waukesha County Notice of Construction Public Informational Meeting All interested persons are hereby notified that the Wisconsin Department of Transportation (WisDOT) will host a Public Information Meeting on Tuesday, Dec. 1, 2009 from 5 p.m. to 8 p.m. at the Town of Ottawa Town Hall, W360 S3337 Highway 67, Dousman. The intent of the meeting is to present information on the WIS 67 pavement maintenance project from WIS 59 to US 18. People having an interest in the project may attend at any time between 5 p.m. and 8 p.m. The meeting will be an open house format. Personnel from WisDOT and the design team will be available to provide information and answer questions on the project, construction schedule, traffic control, and to obtain public input. Construction for this project is schedule for summer 2010. The project involves resurfacing the existing roadway asphalt within its current location, replacing existing roadway signs, and pavement marking. The intent of this project is to address the deteriorated pavement condition along WIS 67. The Town of Ottawa Town Hall is handicapped accessible. A sign language interpreter will be available upon request.
